docs: record B1–B16 remediation in the 2026-06-12 code-audit findings#65

All 16 confirmed bugs are fixed in PR #61 (modern-python/faststream-outbox). Flip status to bugs-remediated, add bugs_pr/bugs_remediated frontmatter, and add a "Remediation status" section: verification result (423 passed, 100% coverage, ruff+ty clean), the implementation approach, the B5 wrong-reason test catch, B13's CI-untestable import path, and what remains open (suspected S1–S5, test-holes T1–T8, improvements P1–P35, doc drift).
