feat: require evidence-rich review matrices

Summary

• Require reviewer acceptance matrices to separate implementation evidence from test evidence.
• Add explicit review flags for silent decisions, scope drift, and missing tests.
• Show the devloop logo at the top of the README instead of the text title.
