[Sprint 14] Harden project board automation reliability

Goal

Improve project board reliability so delivery work moves through Backlog, In Sprint, In Review, Done, and Released with less manual cleanup.

Acceptance Criteria

• Board intake excludes ceremony/meta issues
• Board transitions handle draft, reopen, and unmerged-close PR cases
• Release completion moves eligible Done items to Released automatically
• Milestone review issue creation avoids duplicates
• Board process docs reflect the new automation boundaries
