Pull request creation adding extra files to easily

Our prompting for safe outputs pull request creation is off. I followed what I thought was good prompting and got this:

The problem seems to be the use of git add -A here, though the model then tries to remove files. Still working this out

My original idea was not to prompt the model. Instead just package all changes (staged or not) and create a PR from it.

It looks like when a patch file contains multiple commits, only the first commit is applied

It seems the patch file created in git_patch actually contains multiple commitsm and is in a format to be applied with git am.We have a problem where if there are multiple commits, only the first commit is applied. The problem is likely in the JS processing code for create_pull_request and push_to_branch which both eed to be adjusted and tested for multiple commits
