AzureProvider needs to support serviceVersion to support older service APIs

Describe the bug
The payload blob feature supports configuration of the serviceVersion.

The same is required for the AzureProvider used by bulk export to allow it to interact with an older instance of the Azure blob service (e.g. an older azurite container).

Which version of IBM FHIR Server? 5.0-RC2

Steps to reproduce the behavior:

1. Go to '...'
2. Click on '....'
3. Scroll down to '....'
4. See error

Expected behavior
Add serviceVersion support to the Azure blob bulkdata provider.

I'm going to close this because I needed to add this in order to get export working with my older azurite container. We also test the "not configured" state in the pipeline.
