[Due for payment 2026-06-09] Add suggested followup skeleton loading state

Description

After we finish the "trickled suggested followup" display, we should show "followup skeletons" (loading placeholders) while the next set of suggested followups are generating in the background. This provides a visual indicator to the user that more suggestions are on the way, improving the perceived responsiveness of the followup generation flow.

For the trickle in the video I think we should go 2x the speed.

Here's the skeleton loaders for the buttons:

I think that should be good if it's animating. If we need it to be more obvious then we could add skeleton loaders:

We could also have the thinking paired with the buttons:

Probably prefer something like the last one (maybe just plain buttons no labels though). Cause then there's some context for why the buttons are loading
